110-300-0165(2)(a): A tamper-resistant (TR) cup was observed in TR covers that can pose a choking hazard. 110-300-0165(2)(c): Cords were observed accessible to children in the bathroom near Classroom #2 and in a drawer without a safety lock in Classroom #2. 110-300-0165(2)(d): Plastic bags were observed in every classroom, stored in drawers accessible to children and without a safety lock. 110-300-0165(3)(d): A small refrigerator was observed in the hallway, accessible to children, with no safety lock on it. 110-300-0165(5)(a): An outlet near the sink in the bathroom was observed without a TR cover. Additionally, this outlet does not meet the requirement of WAC 110-300-0165(5)(b): "Outlets near sinks, tubs, toilets, or other water sources must be inaccessible to children or be tamper-resistant and equipped with a ground fault circuit interrupter (GFCI) outlet type." 110-300-0165(5)(e): A surge protector was observed in the outlet near the door in the first classroom, with a child sleeping next to it.
